A 24-year-old clerk and mother of two presented to our hospital in August 2010 with fever, malar rash on the face, vasculitic rash on the fingers and alopecia. Her medical and family histories were unremarkable.

A 70-year-old female has a history of adult-onset asthma managed with a moderate-dose budesonide/formoterol inhaler. In 2019, she presented with skin rash and neuropathy (numbness). Complete blood counts showed elevated absolute eosinophil, peaking at 6,000 cells/μL. In conjunction with skin biopsy and nerve conduction studies, the diagnosis of hypereosinophilic syn¬drome (HES) was made.

Pulmonary arterial hypertension (PAH) is a severe complication of connective tissue diseases (CTDs). At the International Conference of Chinese Rheumatologists (ICCR) 2024, Dr Yeon-Ah Lee of the Kyung Hee University, Seoul, South Korea, emphasized the importance of screening and early detection of PAH in patients with CTDs and provided practical guidance for rheumatologists on screening for CTD-PAH, particularly in patients with systemic sclerosis (SSc) and systemic lupus erythematosus (SLE).

A 20-year-old nulligravida female student, who was a nonsmoker and nondrinker with good past health, first presented in March 2024. She complained of significant alopecia and photosensitive rash that was slightly itchy but not painful, primarily affecting her face and hands for 2 months. She was generally well with no fever or joint pain, and no family history of rheumatological disorders.

A 60-year-old female nonsmoker was first diagnosed with chronic rhinosinusitis with nasal polyps (CRSwNP) and comorbid asthma in 2003. (Table) Due to anosmia (loss of sense of smell), nasal congestion and postnasal drip, she underwent functional endoscopic sinus surgery (FESS) in 2013, which generally improved her symptoms. Tissue biopsy showed abundant eosinophils in nasal polyps. She used daily puffs of a budesonide/formoterol inhaler for asthma control. (Table)

A 39-year-old female hospital porter presented with dyspnoea and Raynaud’s phenomenon in 2010, with no evidence of sclerodactyly. She tested positive for anti-ribonucleoprotein (anti-RNP) and anti–Scl-70 autoantibodies. High-resolution CT (HRCT) of the chest revealed bilateral ground-glass opacities, suggesting interstitial thickening or fibrosis. Echocardiography showed an elevated estimated right ventricular systolic pressure (RVSP) of 74 mm Hg. These findings, along with other defining features, supported a diagnosis of mixed connective tissue disease (MCTD), complicated by interstitial lung disease (ILD) and possibly pulmonary arterial hypertension (PAH).¹

HZ is a debilitating condition that poses a significant burden on patients’ quality of life and the economy and has a bidirectional relationship with cardiovascular disease (CVD). At the 28^th Hong Kong Medical Forum (HKMF), Professor Kai-Hang Yiu of the Department of Medicine, University of Hong Kong, presented an overview of the public health impact of HZ in high-risk patients with immunocompromised conditions or chronic diseases and discussed the long-term effectiveness of adjuvanted recombinant zoster vaccine (RZV) against HZ.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare eosinophilic autoimmune disease that affects multiple organs. The lack of disease awareness and heterogeneous nature of clinical manifestations have made diagnosis and management of EGPA challenging. At Advances in Medicine (AIM) 2024, Professor Hiromichi Tamaki, a rheumatologist from St. Luke’s International Hospital in Japan, discussed how EGPA is diagnosed, presented real-world evidence on mepolizumab (an anti–interleukin [IL]-5 therapy) in EGPA and illustrated mepolizumab’s clinical benefits through several patient cases.
