Caring for patients as a family physician: The heart of holistic healthcare

In the world of healthcare, the role of a family physician is often misunderstood. Patients frequently ask, “Dok, ano po ang specialization niyo?” a common question that I get towards the end of the consultation. My response is equally familiar, “Bakit n’yo po naitanong?”A question answered by another question. Typically, the patient would respond, “Marunong po kasi kayong makipagusap sa pasyente at marami po kaming natutunan.” To which I reply, “Salamat po kung ganoon. Family physician po ako at ganito po mag-alaga ang isang family physician.” This exchange highlights the essence of being a family physician: effective communication, empathy, and clinical competence.

Being a family physician goes beyond simply diagnosing health issues and prescribing medication. It involves understanding the patient in a comprehensive way, considering their emotions, lifestyle, and socio-economic conditions that all influence their health. In this context, effective communication is essential. It helps physicians encourage patients to take an active role in managing their health. By promoting shared decision-making, family physicians support a holistic, person-centered approach to healthcare. This strategy not only addresses immediate health concerns but also supports the overall well-being of the patient.



Family physicians often serve as the first contact point for patients within the Philippine health system. The index patient is our entry point to the family, while the consultation with a family physician is their entry point to comprehensive healthcare services. A family physician’s care extends beyond the confines of the clinic; it encompasses the family and, by extension, the community. It takes into account the family dynamics and functionality, as well as the availability and use of community resources.

Caring for a patient requires a competent and skillful eye that can integrate the context of the patient and the biopsychosocial nature of their health and well-being. This means considering not just the biomedical symptoms but also the psychological and social factors that may affect the patient’s health. A family physician must be adept at deciphering the underlying issues that may not be immediately visible. This approach ensures that the root causes of health problems are addressed, leading to more sustainable and effective health outcomes. Family physicians not just understand the context of the patient, but also the family and how the community can also contribute towards positive outcomes.



One of the cornerstones of family medicine is effective communication. Itis through clear, empathetic conversations that we can inspire patients to be proactive in managing their health. Educating patients about their conditions and involving them in the decision-making process empowers them and fosters a sense of responsibility for their health. Shared decision-making is crucial in ensuring that the management plan is not only medically sound but also aligned with the patient’s values and preferences.

Family involvement is another critical aspect of holistic patient care. Families can be powerful allies in achieving positive patient outcomes. They provide emotional support, help in adhering to treatment plans, and can reinforce healthy lifestyle changes. However, they can also be accomplices in negative health behaviors, making it essential for the family physician to engage with the family as a unit. By involving the family, physicians can create a supportive environment that encourages positive health behaviors and outcomes.

The role of a family physician is dynamic and multifaceted. It involves being a clinician, an educator, a counselor, and an advocate for the patient and family. Itis about building trust and rapport with patients, understanding their fears and concerns, and providing reassurance. This relationship is the foundation upon which effective healthcare is built.

In conclusion, the heart of caring for patients as a family physician lies in the ability to see beyond the immediate medical needs and understand the broader context of the patient’s life. It is about fostering a partnership with the patient and their family, empowering them to take charge of their health, and guiding them through the complexities of the healthcare system. As family physicians, we are privileged to walk alongside our patients on their health journey, offering support, guidance, and care every step of the way. Ensuring health and wellness of a person and the family regardless of ages and life cycle stages.

The next time a patient asks, “Dok, ano po ang specialization niyo?” I hope they leave the consultation understanding that a family physician is a specialist in the art of holistic, compassionate, and comprehensive care and a partner of the family in health. This is the essence of being a family physician, and it is a role that I am proud to fulfill.
