Febuxostat: beyond uric acid lowering Renal, cardiovascular, and metabolic implications

Elevated serum urate is linked to hypertension, insulin resistance, chronic kidney disease, and cardiovascular disease.5 Rather than merely marking disease, hyperuricemia may actively drive these conditions. Recent trials—FEATHER, FREED, and SIRCAR—demonstrate febuxostat slows kidney disease progression in advanced CKD.7-9 Its hepatic clearance makes it ideal for patients with renal limitations who cannot tolerate allopurinol. The CARES and FAST trials confirm cardiovascular safety comparable to allopurinol.10-12
