Direct oral anticoagulants (DOACs) revolutionized AF management, but their safety profiles differ meaningfully.^10-13 Apixaban emerges as the standout DOAC, demonstrating consistently lower rates of major bleeding and gastrointestinal bleeding across trials.^12-13 The ARISTOTLE trial established apixaban's efficacy in 18,201 AF patients, while real-world ARISTOPHANES data on 430,000+ patients confirmed its superior safety profile, particularly in frail populations.^14,16 Recent evidence from ELAN, OPTIMAS, and TIMING trials supports early anticoagulation post-stroke.^17-21 Current ESC guidelines recommend risk-based, patient-centered approaches favoring DOACs over vitamin K antagonists, positioning apixaban as a cornerstone therapy.
