Bridging the gap: Enhancing preventive care in high-risk populations
A recent symposium held in conjunction with the 19th Singapore Public Health and Occupational Medicine Conference convened three experts: Dr Behram Ali Khan, Senior Consultant Nephrologist and Assistant Professor of Medicine, Renal Health Services; Dr Aziz Noordin, Family Physician, Tampines Family Medicine Clinic; and Dr Wee Xue Ting, Cardiology Specialist Pharmacist, iRx Clinical Pharmacy, to examine persistent gaps in preventive care for high-risk populations and discuss practical, scalable solutions. Against a backdrop of rising chronic disease burden, decreasing vaccination uptake, and continued vulnerability among medically complex patients, this symposium highlighted the vital role primary care teams play in modifying risk, improving early detection, and strengthening trust in preventive interventions.
