โรคหรือภาวะที่อาจแสดงอาการคล้ายภาวะอ้วนและควรได้รับการวินิจฉัยแยกโรค ได้แก่:
- Acromegaly
- Adipose dolorosa
- ภาวะท้องมาน (ascites)
- Cushing’s syndrome
- Generalized lipodystrophy
- HypothyroidismIatrogenic
- Cushing’s syndrome
- Insulinoma
- Kallman syndrome และ idiopathic hypogonadotropic hypogonadism
- Polycystic ovarian disease
