Gonorrhea - Uncomplicated Anogenital Infection_Differential DiagnosisA finding of lower abdominal tenderness or cervical motion tenderness should prompt the attending physician to evaluate the patient for pelvic inflammatory disease (PID). Treat the patient accordingly. Other sexually transmitted organisms such as Chlamydia trachomatis, Trichomonas vaginalis, Mycoplasma genitalium, herpes simplex virus, and Treponema pallidum (syphilis) can produce clinical presentations that resemble those caused by Neisseria gonorrhoeae. Various non-sexually transmitted pathogens as well as non-infectious conditions may also lead to urogenital, pharyngeal, or rectal symptoms that may mimic gonococcal infection. Differential diagnoses may also include other causes of acute cervicitis and urethritis, as well as relevant surgical or gynecological conditions.
